Vital Records

The Office of the City Clerk Vital Records Division expanded the locations available to purchase birth records November 2013, to the following Library locations. The certificates sold at these locations will be limited to births occurring from 2005 to present for individuals born in San Antonio.

Vital Records @ Library Locations

Birth certificates (birth dates in 2005 and later) are available at the following branches of the San Antonio Public Library. Please Note: We have been experiencing difficulties printing birth certificates for children born after December 2018. We suggest you call the Library before visiting to confirm that your record can be printed.

Birth Certificates FAQs

Only certificates for people with birth dates in 2005 or later and who were born in the city of San Antonio and Bexar County will be available.

The cost is $23, paid by cash, check, or money order to the City of San Antonio. The fee for a returned check is $30.

Please visit Vital Records, Birth and Death Records Division, at 719 S. Santa Rosa, San Antonio 78204, or call them at 210.207.8815.

At one of the participating branch libraries, check in at the Circulation Desk. Complete a birth certificate application form at the branch. Bring the completed form back to the Circulation Desk and show your identification (all forms of identification must be original). When your certificate is ready, staff will call you by name (usually within 20 minutes). Return to the Circulation Desk to pay and pick up the birth certificate.

State law allows the following individuals to obtain a birth certificate:

  • Individual named on the certificate
  • Related or immediate family member (parent, sibling, spouse, grandparent, legal guardian, or conservator). See list of acceptable documentation to prove relationship
  • Legal representative: Attorney or representative acting under contract bearing a notarized affidavit
  • Name of person you are requesting birth certificate for
  • Person's birthdate 
  • A photo ID of the person requesting the certificate 
  • Proof of relationship

A current valid drivers license or other acceptable photo ID (see complete list).

Immunization Records

Beginning on September 8, 2015 the San Antonio Public Library will no longer be issuing Immunization Records.
